To enable and empower people, primarily those with disabilities, to lead healthy and enriched lives. Our wide range of services under a single umbrella allows us to provide a seamless continuum of care as the people we support age or as their needs change. From intensive medical care to residential services, to dental care to mental health, the individuals we support can have it all managed through the Center.

Catholic Charities of Southeast Michigan (CCSEM) serves the most vulnerable of the community, providing an array of social services in Lapeer, Macomb, Monroe, Oakland, St. Clair and Wayne Counties. Carolina SeniorCare is proud to offer this service to the community. This model of healthcare has been in existence for over 35 years throughout the United States. The program focuses on prevention and wellness with a commitment to enhancing the quality of life for seniors and their families through care and support.
