Heartland Alliance Health’s STAR Program (Supportive Treatment for Addictions and Recovery Team) offers medication assisted therapy (M.A.T.) for participants struggling with opiate use disorder. The program is especially intended for those with dependence on opiates, such as heroin or prescription medication. Individuals must be receiving Primary Care services from Heartland Alliance Health Centers and either have insurance or the ability to pay out of pocket for medication. Participants must also meet the criteria for opiate use disorder as well as agree to safety guidelines.

The Building Stable Communities (BSC) rapid re-housing program provides rental assistance and supportive services for single adults and families in their transition from homelessness to permanent housing and self-sufficiency. Participants live throughout Chicago in scattered-site, privately-owned apartments in their community of choice. The program pays a rental subsidy directly to the landlord, and in most cases the lease is in the participant’s name. To help the participants and their families achieve stability and independence, a range of essential, supportive services are provided, including: housing location/inspection, intensive case management, employment case management, clinical services including mental health and substance use counseling, children’s services, and referrals to other services such as education, primary care and legal services.

Chicago FarmWorks is a program affiliated with Heartland Alliance. Heartland Human Care Services, in partnership with the City of Chicago, NeighborSpace, and the Greater Chicago Food Depository, operates a 2.6 acre urban farm in Chicago’s East Garfield Park neighborhood. It distributes local produce of the highest quality to those who need it most, while supporting people working overcoming multiple barriers to employment to secure full-time, permanent, viable jobs. Chicago FarmWorks also operates a program at Harvest Commons, a supportive housing building operated by Heartland Housing. Residents participate in nurturing the garden, a flock of chickens, and honeybees. By locating affordable housing and a sources of fresh, healthy food onsite, we are addressing the health disparities facing many of Chicago’s low-income communities.

Allendale County Mental Health Clinic is operating under the auspices of the Coastal Empire Mental Health Center; its primary mission is to serve clients with serious and persistent mental illness. Major insurances are accepted, including Medicare and Medicaid. Staff are available to discuss payment plans for clients and their families.
