The Supportive Permanent Housing Program is a community housing program affiliated with Heartland Alliance. It houses eighteen individuals who are homeless and who suffer from a serious mental illness who may have a co-occurring substance use disorder. Supportive services are designed to enhance activities of daily living and problem-solving skills, promote independence and dignity, improve satisfaction, and ensure success in community living.

This is a federally-supported Community Health Center/School-Based Health Center, providing physicals, health education, counseling, and laboratory services. They treat adults and children including EPSDT (Early and Periodic Screening, Diagnoses and Treatment Program) for Medicaid eligible children, and immunizations. This facility includes a pharmacy. All Community Health Centers accept Medicaid, Medicare and most private insurances and offer a sliding fee discount program for eligible low income families. Our community services department provides numerous programs related to after school tutorial services, fine arts, recreational activities, scouting, seasonal camping, primary prevention leadership programs for school aged youth.
