Senior Care Partners P.A.C.E. (Program of All-Inclusive Care for the Elderly) was formed in conjunction with the National PACE Association to provide comprehensive care for the eldery through an integrated team of healthcare professionals. Services offered include but are not limited to primary medical care, a variety of physical, social, and emotional wellness programs, home health care and home-delivered meals, inpatient services such as hospital and nursing home care, medication monitoring, medical specialist availability, and various outpatient services, respite services, as well as a day center for elderly adults.

The Severe Weather Network (SWN) partners with community organizations, agencies, and faith communities to provide emergency, overnight refuge during the winter months for people experiencing homelessness in Livingston County.
